Forward Head Training: 80/20 Protocol¶
Câu hỏi gốc¶
What if I train with a forward head on purpose — would that build better balance?
1. Sensory reweighting¶
- Mất horizon → brain up-weights vestibular + ankle proprioception
- Nghiên cứu: vestibular training enhances multisensory integration in postural control (Sensory Organization Test), kể cả healthy adults
- Bạn dạy hệ thống giữ thẳng khi vision unreliable
2. Habituation to pitch¶
- Bending forwards = standard habituation drill
- Provoke dizziness, let symptoms settle, repeat, progress
3. Specific carryover¶
- Stroke rehab: induced forward-tilt training restores ambulation in patients stuck in backward retropulsion
- Parkinson's: vestibular-oriented balance work improves postural control hơn traditional balance work
→ Nếu sport/life yêu cầu nhìn xuống (wrestling, BJJ, MTB, plumbing dưới bồn) → ngắn + controlled forward-head practice giúp bớt hoảng khi mất horizon
Cái gì tệ đi nếu bạn sống luôn ở đó¶
(Đã note từ 05.1_Head_Position_Control_Loop: "no bend of the knees")
- VOR tries to stabilize vision during head movements by adjusting eye muscles, but hits its limit → Dynamic visual acuity drops, miss fast cues
- Brain chọn stiffness over fluidity:
- Healthy people thực ra có head movements biến thiên nhiều hơn vestibular patients
- Higher sample entropy = more adaptability, không phải less
- Tự tập lock head + lock knees = tập ngược lại adaptability
- Neck extensors fatigue, thoracic flexion increases, baseline neutral calibration drifts
That's why rehab frameworks never start with forward tilt Họ progress qua 6 categories:
- static
- compliant surface
- weight shifting
- modified center of gravity
- gait
- VOR
|---|---|
| 80% balance work | Neutral head, horizon available (build VOR gain + ankle strategy) |
| 20% | Deliberate horizon removal (short, progressive, measurable) |
4-step progression (từ clinic protocol)¶
1. Seated pitch holds¶
- Sit, tuck chin 20° down, hold 30 sec while breathing
- No sway allowed
- 3 reps
- Habituation, not strength
2. Standing weight shifts, nose down¶
- Feet hip-width, pitch head, shift weight slow side-to-side
- Keep knees soft on purpose — override the lock reflex
- 1 minute
3. Gait với brief look-down¶
- 10 steps neutral → 3 steps looking at feet → lift
- Repeat
- Training transition, not tilt itself
4. Eyes-closed variant¶
- Same as #2, but eyes closed for 10 seconds
- Removed both horizon and detailed vision
- Force vestibular + proprioception
Stop rule¶
Nếu lingering dizziness, nausea, neck pain → not adaptation, that's overload → back off to neutral
Bottom line¶
Train forward head on purpose = builds specific kind of balance (stability without horizon)
Nhưng KHÔNG tạo better balance overall nếu biến nó thành home base
Use như spice: small doses force vestibular system to work → return to neutral to integrate gains
Head leads, but should lead back to level most of the time
